A seventy four-year-old male came to our hospital for abdominal pain, cough and mild fever, 37.0 to 37.5 centigrade. Laboratory tests revealed elevation of white blood cells. Then, he was given intravenous antibiotics twice a day for suspicious diverticulitis. Despite antibiotics for five days, mild fever was persistent. He had been experiencing persistent cough for approximately two months. He was advised to take admission for further examination. Arterial oxygen saturation (PO2) was 94 mmHg at rest and 88 mmHg after walking. Laboratory test revealed CRP 3.15 mg/dL, white blood cells 12400/mm3, neutrophils 75.8 (40-71) %. He agreed to receive CT again for further investigation.
