A seventy three-year-old female was transported to our hospital by ambulance car. She was found to be falling into sitting on the floor and leaning to food shelf in a supermarket. When a store stuff talked to her, she was in a restless state with vague response. Medical findings revealed blood pressure 189/119 mmHg, pulse 115/min, positive Barre sigh on right, positive Mingazzini sign on right and dysarthria, indicating right hemiparesis. She was routinely given medicines for hypertension and cardiac fibrillation. She took MRI for further investigation (Figs. 1, 2).
