We present a sixty nine-year-old male for swollen right lower leg. He worked for traffic control. Ten days ago, he served as a guard for traffic security and worked standing for very long without a rest. He realized painful right leg but put up with it and kept running around for traffic control. The following day, right swollen leg and thigh with purpura emerged. He visited a local clinic and received CT (Figs. 1-3). He was introduced to our hospital. Laboratory test revealed white blood cells 7090/mm3, red blood cells 3830000/mm3 CRP 1.71 mg/dL, Hb 12.8 mg/dL, Ht 37.4%, platelets 142600/ mm3 , CPK 203 U/L, fibrinogen 485.2mg/dL, D dimer 8.3μg/mL, LDH 253 U/L.
He received MRI for further investigation (Figs 4-6).
