We present a seventy two-year-old male with cough and high fever of 38.3 ℃. He had received surgical resection for sigmoid colon cancer approximately two years ago. Thereafter, he was given chemotherapy for distant metastasis. As the first line, C-mab plus FOLFOX were administered and as the second line, FOLFIRI were followed and finalized just one week ago. Laboratory test revealed CRP 6.53 mg/dL, white blood cells 7200/mm3, neutrophils 60.5%, lymphocytes 17.9%, monocytes 12.5%, platelets 120000/ mm3. He took chest CT for further investigation (Figs 1-3).
