A Case of the 10 days

Case 241

 A seventy two-year-old female presented in our hospital for back pain with her son. She was carried by a stretcher. She experienced sudden abdominal pain. The pain moved to right lower abdomen to back pain. She went to a local clinic for hypertension. Medical examination revealed distension of abdomen, no bowel sound, muscle defense, and hardness of upper abdomen. The present illness history and medical findings were suspected to arise from perforated digestive organ. Immediately before entering CT room, she fell into pre-shock state. Under oxygen inhalation, she took CT for further investigation (Figs 1-3).

What is your imaging diagnosis ?

1. Digestive organ perforation

2. Cardiac tamponade from cardiac infarction

3. Cardiac tamponade from aortic dissection

4. Acute pericarditis

5. Acute myocarditis
