Case 232

 A sixty six year-old male presented in our hospital for lower abdominal pain. He experienced abdominal pain in the midnight and/or before dawn once per three days. He consulted with the physician in a local clinic where he received abdomen echo, revealing no abnormality. He got some medicine from the clinic but in vain, lower abdominal pain was repeated. Laboratory test revealed no elevation of inflammatory values of white blood cells (7820/mm3) and CRP (0.03 mg/dL).
 He took non-enhanced CT and a few hours later contrast-enhanced CT for further investigation (Figs 1 to 4).

What is your imaging diagnosis ?

1. Inflammatory aneurysm

2. Penetrating atherosclerotic ulcer

3. Aneurysm rupture

4. Aortitis syndrome

5. Aortic occlusive disease
