Case 228

 A sixty five-year-old male presented in our hospital with slight fever and hypoxia. Slight fever occurred seven days before and the following day, PCR test was positive, implying COVID19. He stayed at the designated lodging but hypoxia emerged yesterday, asking a favor of admission to our hospital. Laboratory test 7 days vs 13 days after the onset of slight fever revealed white blood cells (mm3) 4010 vs 8380, neutrocytes (%) 72.2 vs 87.5, lymphocytes (%) 18.1 vs 8.3, monocytes (%) 7.7 vs 2.4, platelets 129000 vs 83000, D dimer (microgram) 1.3 vs 76.6. He received CT 7 days and 12 days after the onset of COVID 19 (Figs 1 to 3).

Why do D dimer values suddenly elevate at Day 13 compared to at Day 7 ?

1. Bacillus infection

2. Fungus infection

3. Pulmonary branch artery thrombosis

4. Lung edema

5. Inhalation pneumonia
